Preparing for your First Visit

Your first visit with me is the beginning of your personal healing process and the foundation of the trust, honesty and mutual respect necessary for a sacred therapeutic relationship.

For our work together to be most effective, please fill out the Patient Health Profile as completely and truthfully as possible prior to your first visit. Your thoughtful and candid answers on the Profile and during our visit provide me with a deeper understanding of your health concerns and serve as the basis for my initial treatment recommendations.

Phone consultations and home visits are available for those who are unable to find convenient, qualified and licensed naturopathic medical care in their community, are physically unable to come in to the office, or simply prefer the comfort and privacy of their own homes. Please call our office if you are interested in these special services.

Before your first office visit or phone consultation, please mail or fax the following to my office:

• Your completed Patient Health Profile.
• Any recent laboratory and/or blood work results.

Bring to your first visit: (please fax this information if it is a phone consultation)

• All medications and/or supplements you are taking and dosage amounts.

At your first visit, you can expect that I will:

• Review your Patient Health Profile and lab results.
• Obtain a comprehensive health history and diet and lifestyle assessment.
• Listen compassionately to your health concerns.
• Perform a physical examination.
• Recommend a nutritional program and discuss the spiritual connections to your health concerns.
• Prepare and outline goals for your next appointment.

Visit Types

Dr. Tom is available for in-office visits and extensive phone appointments. Please call the office nearest your location to schedule.

Special in-person consultations can be arranged for patients worldwide who wish to see Dr. Tom in the comfort and privacy of their own homes.
